When enrolling your child, please follow the process below. Enrolment documentation must be completed by the student's parent or caregiver.
Enrolment Process
Step 1 – Check your catchment
Check the Queensland Government EdMap – School Catchment Maps to confirm that your residential address is within the Noosa District State High School catchment area.
Step 2 – Complete the enrolment forms
Please review our Prospective Student Enrolment Information Booklet to learn more about Noosa District State High School and the enrolment process. When you are ready to proceed, complete and submit the NDSHS Enrolment Package below.
Please provide the following supporting documentation with your application:
- Birth certificate
- Proof of residential address, including:
- Primary source – current lease agreement, unconditional purchase agreement or rates notice showing the parent/caregiver's name and residential address
- Secondary source – current electricity or gas utility bill showing the parent/caregiver's name and residential address
- Latest academic report
- Custody or parenting documentation, where applicable
- Relevant medical, health, disability or additional support information, where applicable

Alternatively, you can begin the enrolment process by completing the relevant digital enrolment form and visit the appropriate campus:
- Pomona Campus – Years 7–8
- Cooroy Campus – Years 9–12
Proof of Residency
Where a family resides within the school catchment but is unable to provide the standard proof of residency documentation, enrolment cannot proceed until alternative evidence has been provided and eligibility to enrol has been considered by the Principal.
This may include:
- A statutory declaration from the parent/caregiver confirming that the identified address is their principal place of residence; and
- A statutory declaration from the person whose name appears on the relevant proof of residency documents.
Additional Information
Previous school reports and other relevant educational records may assist the school with appropriate class placement and planning for your child.
If your child requires adjustments or additional support, please provide any relevant reports from specialists, medical practitioners or other professionals. This information assists us in planning appropriate support for your child.
If your child requires medication to be administered at school, the appropriate medication forms must be completed. Medication must be supplied in its original packaging with a pharmacy label containing clear instructions for administration. Further information about medication requirements is available from either campus office.
